How this Works
You approach local primary care physicians (MD, DO, NP, PA) and propose a partnership.
Your pitch: "I'll help you manage your chronic disease patients remotely. You'll improve outcomes, increase revenue, and reduce your workload. I'll bill insurance and pay you a percentage."
What physicians get:
• Better patient outcomes (less ER visits, better A1C, lower BP)
• Additional revenue ($50-$75 per patient per month)
• Less phone calls ("Can you refill my medication?")
• Minimal effort (you do all the work)
What you get:
• Access to their patient panel
• Ability to bill under their NPI
• Recurring monthly revenue
• Clinical autonomy
The physician refers patients to you. You call them, explain the service, get consent, enroll them in your EHR.
Ideal patients:
• 2+ chronic conditions (diabetes, hypertension, COPD, CHF, etc.)
• Medicare or Medicaid (best reimbursement)
• Stable but need ongoing management
• Willing to do monthly phone/video calls
You'll enroll 10-20 patients per physician office.
You call each patient monthly (20-30 minutes per call).
What you do:
• Review medications (adherence, side effects, interactions)
• Review vitals (BP, glucose, weight)
• Coordinate with physician (send recommendations, adjust meds)
• Document time spent in EHR
• Bill insurance monthly
Time commitment: 20 minutes per patient per month = 30 patients = 10 hours/month
You bill Medicare/Medicaid/commercial insurance using these codes:
CCM (Chronic Care Management):
99490: $45-$65 per patient per month (20+ minutes)
99439: Additional $35-$50 (20+ additional minutes)
RPM (Remote Patient Monitoring):
99457: $55-$75 per patient per month (20+ minutes)
99458: Additional $45-$60 (20+ additional minutes)
PCM (Principal Care Management):
99426: $70-$90 per patient per month (30+ minutes)
99427: Additional $55-$70 (30+ additional minutes)
Average revenue: $80-$125 per patient per month
• Month 1-3: 1 physician, 10-20 patients, $1K-$2K/month

The Pivot Program

Months 1-3: 5-10 hours/week (business setup, physician outreach)
Months 4-6: 10-15 hours/week (managing 30-50 patients)
Months 7-12: 20-30 hours/week (managing 100+ patients)
You can do this part-time while keeping your day job.
I teach you 5 different outreach strategies. You'll approach 20-30 physicians in your first 60 days. Based on my members' experience, 10-20% convert. That's 2-6 signed contracts.
If you approach 5 physicians and stop, you'll fail. If you approach 50 and adjust your pitch, you'll succeed.
Medicare and Medicaid cover these services. I teach you how to bill correctly, handle denials, and work with billing services. My members have 85-95% clean claim rates.
No. Most members start part-time. Once you hit $5K-$10K/month, you can decide if you want to go full-time. Some members scale to $20K/month while still working part-time.
You'll get word-for-word scripts, email templates, and role-play practice. Most members say "I was terrified of sales" on Day 1. By Day 90, they have signed contracts.
All 50 states. Some states require collaborative practice agreements (CPAs), some don't. I'll show you exactly what's legal in your state.
Yes, RPh licenses work too. You'll need to check your state's scope of practice, but most states allow RPhs to provide these services under a CPA.
